Transitional Fall Prevention

You may be caring for someone who has been discharged after a fall or surgery, is not yet well enough to return home. or is moving in with you permanently.  Visit us at our donation center for hands-on assistance in getting your home ready for them!

Stop Falls Program

Falls threaten the independence of older adults and those with limitations. However, falls do not have to be inevitable as you age or are living with a disability. There are proven ways to reduce and prevent falls.

Share the Care Napa Valley’s fall prevention program provides screening, home safety assessments by a licensed occupational therapist, and home modifications as well as assistive device installation by a contractor at NO CHARGE to Napa County residents.

Click below to get to our referral form. We prioritize those with the greatest socioeconomic need and who are at a high fall risk.

Declutter Program

We work with Napa County residents to create healthy and safe living environments by removing excess clutter that impacts the quality of life. Often serving those facing homelessness, limited mobility, or other disabling conditions. Share the Care Napa Valley coordinates haul-aways, picks up reusable items, deep cleans residences, and connects clients with necessary support resources FREE OF CHARGE.

Declutter Lifestyle Coaching

After we help you meet your decluttering goals, we continue to visit and assist you in maintaining your new, clutter-free lifestyle. We provide ongoing support to ensure your environment remains healthy and safe.

*Clients must first complete our Declutter Program for coaching
